Yes. All patients are required to provide us with proof of medical and vision insurance, i.e., an insurance card and his or her drivers license or state I.D. at the time of their appointment. Because insurance information changes or is updated – by the patient or the provider – providing us with your information allows our staff to provide you with your accurate medical coverage and co-payment information. At Vision Health Center, we do not keep insurance information on file for the following year’s appointments. This information is required to be updated every year at the time of your appointment.
